Social Worker

An opportunity has become available for a qualified social worker within the Transitions Service, based at St Mary's House, Eastbourne. This is an exciting opportunity to work across both Children's and Adults Services supporting young people, their families / carers during transition. The Transition Service has been set up to provide a route through from Children's Disability Services to Adult Social Care for young people aged 16-25 with the most complex needs.   Positive outcomes for clients is our top priority. But we know that to achieve this, you need the support and professional development to make the most of your skills.
