An exciting opportunity has arisen for a Social Worker/Resource Officer with the Integrated Continuing Healthcare Team. You will be part of a large innovative team of Social Workers, Resource Officers and Nurse Assessors based in Limousin House in Eastbourne. Adult Social Care is experiencing an unprecedented level of change with the implementation of the Care Act, closer integration with Health as part of ‘Better Together’, the introduction of a New Social Care Information system and ‘Agile’ working. All of these key change programmes impact on the Adult Social Care. 

The team undertake Continuing Healthcare eligibility assessments and reviews countywide and therefore applicants will need to have access to suitable transport.
