About the Role

An exciting opening has arisen for a social worker to cover maternity leave for 12 months for the Transition Service.  The team supports young people 16 – 25 with disabilities to move into adult services in a planned and personalised way.  You will receive on-going training, support and guidance in all aspects of your role including working with Looked After Children held by the team.  If you are positive about enabling young people to move into adult services in a person centred way and have a creative approach to support planning then we want to hear from you. Positive outcomes for clients is our top priority. You will need to be a driver / car owner as we are a countywide team.  

This is a wonderful opportunity for you to enhance your CPD and to work across services you may not have had the opportunity to do so in the past including Children Services, Education, Adult Social Care, CAMHS, Continuing Care and the CHC team. 

You will need to seek agreement from your line manager prior to applying if you wish to take up this opportunity as a secondment.  Otherwise, it is a fixed term one year contract.

This post is subject to an Enhanced DBS check or Enhanced check for Regulated Activity with the Disclosure & Barring Services (previously the Criminal Records Bureau). Please note that we require evidence of an overseas police check if you have lived or worked abroad.

Your starting salary will be pro rata if the above position is part-time or term-time only.

Work Permits: we may be able to obtain a Work Permit for this post but this is subject to meeting the requirements of the UK Border Agency’s Points-based Immigration System.
