About the Role

The Transitions Service support young people from the ages of 16-25 with disabilities to move into adult services in a planned and personalised way. We are looking for a motivated and proactive qualified Social Worker who is committed to ensuring that the service supports young people and their families to make choices about their future that are individual, timely and appropriate to join our growing team. You will need to have recent experience of children’s social work and a thorough knowledge of relevant legislation relating to children with disabilities.

This post is subject to an Enhanced DBS check or Enhanced check for Regulated Activity with the Disclosure & Barring Services (previously the Criminal Records Bureau). Please note that we require evidence of an overseas police check if you have lived or worked abroad.

Your starting salary will be pro rata if the above position is part-time or term-time only.

Work Permits: we may be able to obtain a Work Permit for this post but this is subject to meeting the requirements of the UK Border Agency’s Points-based Immigration System.
